FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics, and can be used for replacement of ankle defects in experiment. Shin and foot are fixed in an external fixation apparatus. There are two crossed pins passed through the tibia in the middle one-third, which are fixed in the base ring. Two intersecting pins are inserted through the calcaneal bone and fixed in the base semi-ring. Then, on the border of the middle and lower one-third of the shin, two crossed pins are inserted, which are fixed in the stabilizing ring. Rings are interconnected by means of rods. That is followed by an anterior long-length approach to ankle joint 6–8 cm long along a medial edge of the anterior tibial tendon starting 4–5 cm above the joint space line and 3–4 cm below it. Proximal extensor extensor is Z-shaped to cross the anterior tibial artery, the vein and the deep branch of the fibular nerve together with the tendons of the long extensor of the thumb and the long extensor; then preparing a bed comprising a body and an ankle block or, in its absence, a posterior articular facet of a calcaneal bone for a transplanted graft by bone or cartilage resection. That is followed by exposing the distal shin bone metaepiphysis and using an oscillating saw to form a bone bicortical transplant in the form of a truncated pyramid. Medial approach to tibia 3–4 cm long is performed. Transplant is corrected by the osteotomy of the posterior edge of the distal shin of the tibia. That is followed by a posterior approach along an Achilles tendon of 2.5–3 cm; after creation of bicortical graft, a pin is inserted through it, which is fixed in a distraction ring; then transplant is moved into prepared bed and fixed with preliminary restoration of height of rear foot and bringing foot into optimal position.
EFFECT: method provides experiment creation of a method of treating large ankle defects, allowing to provide conditions for tibial calcane arthrodesis with preserving anatomical relationships in the distal area of the shin and foot due to reliable retention of ankylosized surfaces of tibia and heel bone in a given position.
