SUBSTANCE: method is performed using a mucoperichondrium flap from one side and a mucous membrane flap from the inner cheek surface with a medial base on the other side, for which the nasal septum skeleton is subperiosteally and subcartilagely isolated under endoscopic control, perforation edges are deepithelialized, twisted cartilaginous fragments are partially resected, mobilized and set along the midline, a mucous membrane flap is isolated on the inner cheek surface on a pedicle with a left medial base, a tunnel formed in the anterior nasal spine, the flap is directed between the layered mucoperichondrium perforation edges on the left side, straightened and sutured from the left under endoscopic control, right perforation after local tissue displacement is sutured, silicone splints are fixed to the nasal septum.

EFFECT: method allows to improve the reliability of nasal septum perforation plastic closure and reduce postoperative recurrence.
