SUBSTANCE: invention relates to plastic and maxillofacial surgery. Performing subperichondrium and subperiosteal detachment of soft tissues from lower and upper lateral cartilages to root of nose. Lateral and medial pedicles of greater alar cartilages are removed. Further, a costal autocartilage is used to form a rectangular autograft with thickness of 2 mm and height of 20 mm to restore the lost part of the nasal septum. Autograft is sutured to the prepared caudal portion of the nasal septum with a continuous eight-shaped suture in four vertical turns with a non-absorbable monofilament synthetic suture. Further, the medial legs of the wing cartilages are pulled up to the autograft and anchored with a U-suture with a non-absorbable monofilament synthetic suture. Mucocutaneous flap is put in place and closed with interrupted synthetic monofilament suture. Intranasal splints are inserted into the nasal cavity and fixed with one interrupted suture. Plaster is applied on the nose. External thermoplastic nasal splint is fixed.

EFFECT: method enables reconstructing the structures and restoring the lost anatomy, length and projection of the terminal portion of the nose in the patients with postoperative deformities of the osteochondral portion of the nose.
