FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely traumatology-orthopedics and neurosurgery, and can be used for surgical management of lumbar spinal cord. It involves partial removal of a yellow ligament in the medial part of the intercostal space above the dural sac preserving the integrity of the yellow ligament and periosteum in the lateral sections above the spine. Fragment of fatty tissue is a fragment of epidural fat, its displacement in caudal and cranial directions with preservation of vessels with feeding legs. After the herniation of the intervertebral disc is removed, the epidural space is filled around the backbone and dural sac with anti-adherent gel in amount of 2–3 ml. Fragment of epidural fatty tissue is expanded and filled with a yellow ligament defect, as well as a dorsal surface of the backbone and a dural sac. Second layer of anti-adhesive gel is applied on the epidural fragment of fatty tissue over the defect of the yellow ligament in amount of 1–2 ml, restricting muscular tissue.

EFFECT: invention provides more effective prevention of scar and adhesive process by reducing traumatization of osteoligament and dural structures of epidural space, preservation of barrier functions of natural anatomical formations in combination with application of anti-adhesion materials.
