FIELD: medicine; neurosurgery.

SUBSTANCE: invention can be used for prevention of epidural fibrosis in revision biportal endoscopic discectomy. Method involves interlaminectomy, adhesiolysis, meningoradiculolysis. X-ray marking of the access trajectory is performed in the projection of the lower edge of the upper arch and the upper edge of the lower arch. Linear skin incisions 9–10 mm are made. A working instrument port and an endoscope port are formed. Endoscope is inserted into an upper port; a medial facetectomy is performed through a working port under constant irrigation of normal saline. Endoscope-controlled adhesiolysis, meningoradiculolysis with mobilization of nerve root are performed. At the end of the operation, after stopping the supply of physiological solution under control of the endoscope through a free port with the help of a nozzle attached to a syringe with anti-adhesion gel. Gel is delivered in amount of 5 ml to the area of neural structures in the area of operation. If observing a disc herniation, it is removed.

EFFECT: method provides prevention of epidural fibrosis during revision biportal endoscopic discectomy, regression of pain syndrome due to application of anti-adhesion gel in conditions of manifested phenomena of spondyloarthrosis, osteoarthrosis with minimization of surgical trauma.
