Abstract RU 2711257 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to pediatric surgery, and can be used for treating lymphangioma of soft tissues in children. Video endoscopy equipment is used for this purpose. Three skin incisions of 3.5–4 mm are performed, and complete lymphangioma is recovered from surrounding tissues using endoscopic instruments: three trocars No. 3.2; 3.2; 3.5 mm, endoscope, dissector, and fenestrated forceps. At that, after skin incision and installation of the first trocar in subcutaneous fat, an artificial small working space is created by injecting gas CO2 to pressure of 5–6 mm Hg with flow rate 7 l/min. It is followed by pneumatic dissection of soft tissues surrounding lymphangioma within subcutaneous fat, preliminarily delimited by contracting by elastic pressure-tight adhesive tape adjoining on skin periphery, at 1 cm from the borders of lymphangioma projection. That is followed by inserting a camera and placing endoscopic instruments under visual control. Lymphangioma is recovered from soft tissues with preservation of its structure. After complete discharge and emptying, lymphangioma is extracted through one of the incisions for endoscopic instruments. Tubular drainage is inserted into the lymphangioma bed through a lower trocar, and fixed to the skin with an interrupted suture. Trocars are removed from subcutaneous fat. Wounds are closed. Pressure bandage is applied on the skin of the postoperative area.

EFFECT: method provides complete radical removal of lymphangioma of soft tissues, prevention of lymphorrhea in the postoperative period, improved cosmetic effect of the operation, enables to exclude recurrent formation.
