FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics, and can be used for stage surgical treatment of comminuted fractures of thoracolumbar spine. At the first stage, a medial dorsal approach to the spine is performed with skeletonisation of the posterior structures of the injured, above- and underlying vertebras, installing a transpedicular fixation system by inserting bilaterally transpedicular screws adjacent to the injured vertebrae, interconnected by means of rods and nuts with distraction and spinal lordosation. Post-traumatic kyphotic deformation is corrected and the fragments of the injured vertebra are reduced, to which a guide is then transpedicularly inserted to insert a material filling the cavities between fragments of the injured vertebra. Material used is bone cement with provision of additional attachment of fragments of the injured vertebra to each other. Second stage of surgical treatment is performed in 2 months, at this stage, a resection of the damaged vertebral body reinforced with bone cement is performed through anterolateral access to form a bone bed and a mesh implant filled with osteoinductive material is inserted into it.
EFFECT: method provides complete attachment of all fragments of damaged vertebral bodies to each other, providing their primary stable fixation with subsequent preservation thereof both during activation of the patient, and in a remote period by eliminating at the second stage of the surgical intervention the factors leading to instability of the spinal column.
