FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to maxillofacial and reconstructive plastic surgery, and can be used for reconstruction of surface defects of soft tissues of face and oral cavity. Multispiral computed tomography, high-frequency Doppler ultrasound are performed, and the size of the free skin-fat perforating flap required for defect closure is determined. Receiving bed is prepared by incision of skin and subcutaneous fat of recipient area with mobilization of soft tissues of recipient area, recipient vessels are separated within a defect; preliminary markings is performed and two semilunar incisions are made from a lateral edge of an inguinal tubercle to an anterior iliac spine, and the flap is dissected. A perforating vessel and comitint veins are visualized and separated at the level of a superficial abdominal fascia. A free skin-fat perforating flap is separated and modelled according to the defect, legation is performed at the base of the vascular pedicle; then, the vascular pedicle is dissected away, and the flap is lifted with irrigation of the donor artery and vein. On the donor vessels of the flap, vascular clips are applied, a free skin-fat perforating flap is taken and adapted with interrupted sutures in the recipient area, the wound is closed in parallel in the donor area, further, anastomoses are applied with recipient vessels and a free skin-fat perforating flap is revascularized using terminal branches of the main arteries or anastomoses, thereafter, the revascularized free skin-fat perforating flap is finally adapted and fixed in the defect area with interrupted sutures.

EFFECT: method allows to increase functional and aesthetic result, reduce injuries and probability of complications and to provide the recipient area with the necessary blood supply due to the combination of using the thin free skin-fat perforating flap on the superficial artery and using the terminal branches of the main arteries for anastomoses.
