SUBSTANCE: invention refers to medicine, namely to thoracic surgery, and can be used for elimination of anterior thoracic wall defect. Method of fixing a muscular flap for eliminating a residual cavity in an anterior mediastinal segment involves cutting a muscular flap, moving it on a front thoracic wall and fixing. Fixation of the flap is performed with nodal absorbable ligatures, applying horizontal U-shaped sutures at 3 cm intervals. Needle is inserted from the outside into the thoracic fascia with prick out through the muscle edge. Then, needle prick in muscle graft is medial from point of prick out by 2 cm, after 5 mm again prick out. Then 5 mm is lowered prick into muscle with prick out through 5 mm and then through 2 cm prick into muscle edge with prick out through thoracic fascia. Perforated drainage is placed under the muscular flap and brought out through the skin in the lower part of the wound. Nodes are tightened so that a portion of the muscular flap located between the lateral and medial prick of the needle is displaced to the wound bottom.

EFFECT: method provides prevention of muscular flap ischemia development, as well as avoiding ligature eruptions by making a suture with distance of 2 cm between pricks in the muscular flap in the transverse direction.
