SUBSTANCE: invention relates to medicine, namely to thoracic surgery, and can be used to repair extensive defects of the anterior chest wall after postoperative sternomediastinitis. After extirpation of the sternum within the healthy tissues of the cartilaginous joints with the removal of the xiphoid process, the size of the formed defect of the chest wall is determined. Next, the flap of the large omentum on the pedicle is mobilized, corresponding to the size of the formed defect. In the bed of the xiphoid process, a channel is formed through the diaphragm, through which the omentum flap is moved to the anterior chest wall and fixed to the edges of the chest wall with nodular sutures. Then the anterior chest wall is covered with a polypropylene mesh implant with an overlap on the ribs and fixed. The flaps of the greater pectoral muscles are mobilized and a “coat” type duplicate over the defect zone is formed.

EFFECT: method makes it possible to eliminate the residual cavity in the anterior mediastinum, to stop the phenomenon of sternomediastinitis, provides stabilization of the chest and prevention of postoperative seromas due to the combination of techniques of the claimed invention.
