METHOD FOR SURGICAL MANAGEMENT OF CHRONIC ANTERIOR DISLOCATIONS OF SHOULDER Russian patent published in 2020 - IPC A61B17/56 

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and is used in surgical management of long-term anterior shoulder dislocations. Deltoid-pectoral approach to the shoulder joint is performed, and a coracoid process is recovered and mobilized with retention of a short head of the biceps, coracohumeral and small pectoral muscles. Osteotomy of the coracoid process at its base is performed. Tendon of the subscapular muscle is cut off from the small tubercle. That is followed by correction of chronic anterior dislocation and transarticular fixation of head of humerus by K-wires to articular process of blade. Subscapular muscle is delivered through a cleft in a tendon portion of a small pectoral muscle; the coracoid process is fixed in the zone of the defect of the blade with screws and the tendon of the subscapular muscle is fixed to the small tubercle of the humerus.

EFFECT: method provides higher stability of the injured shoulder joint by reconstructing the strength properties of the tendon-muscle complex of the subscapular muscle, reducing the probability of the recurrent dislocation, as well as improved congruence of articular surfaces of shoulder joint, including due to replacement of defect of anterior part of articular process of blade, restoration of anatomical area of fixation (punctum fixum) of subscapular muscle without additional elongation of its tendon.
