PATHOANATOMICAL EXPOSURE METHOD Russian patent published in 2020 - IPC A61B16/00

Abstract RU 2734747 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, particularly to pathological anatomy. In pathoanatomical exposure, a median, straight incision of the skin and underlying soft tissues from the point below the chin is performed at 3, 4 cm to the jugular notch. Cutting direction is changed from median line at level of jugular notch to the left at angle of 90° at level 2 of the intercoastal space to the anterior or middle, or posterior axillary line, continue to 10th rib inclusive to the point near the transition point of the cartilage part into the bone, after which the incision is continued to the xiphoid process, then the skin of the selected axillary line is incised downwards, the sawing is performed at the junction of the bone and cartilaginous parts of the ribs, the front part of the thorax is lifted, the left pleural cavity is inspected; articular capsule of both sternoclavicular joints is dissected and clavicles are dislocated; thereafter, the sternum is separated from the inner side from the soft tissues of the anterior mediastinum, then the left and right half of the diaphragm is mobilized to their feet, I-X ribs are incisionally resected; intra-I-X incisor is transected subperiosteally, wherein intersectional line of ribs following one another passes from the right sternoclavicular junction from top to bottom, mainly in the area of transition of the bone portion of the rib to the cartilaginous tissue, after which the bone frame of the anterior portion of the thorax is lifted from the left and discharged by 180° in right side of body, wherein the hinge is the mobility of the anterior thoracic segment along the line of the subperiosteal intersection of the ribs from the right, performing the complete neck and thoracic cavity, abdominal cavity and organs of retroperitoneal location with a single unit, to restore continuity of ribs on the left in places of previously made intersections ligatures or metal brackets are used.

EFFECT: method provides complete restoration of anatomical shape and frame properties of the chest, achieving cosmetic effect, reduces body injuries, eliminates random displacement of the resected portion of the thorax by preserving skin with subcutaneous fat, a muscle case, an aponeurosis on the right, as well as strong fixation of the ribs on the left.
