FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to ophthalmosurgery. Scleral flap is fixed while performing antiglaucomatous operation with implantation of EXPRESS® drainage system. Trapezoidal scleral flap is formed with a base to the limb with size of 6 mm in the base, with height of 7 mm and 3 mm in the apex of the flap, with thickness equal to 1/2 of the total thickness of the sclera. Flap is separated at depth of 1/2 of the scleral thickness towards the limb until the corneal tissue develops, at distance of 1/3 from the base of the scleral flap in the scleral bed with a blade, two incisions are made to the vascular cover parallel to the limb on the entire width of the scleral bed with a distance between incisions of 1.2–1.5 mm. Thereafter, the tunnel is formed under the incisions thus separating the scleral bed strip separated from the choroid. After installation and fixation of the drainage system, the branches of the capsular forceps are carried out under the formed band of the scleral bed, being introduced from the distal side and brought out through the medial one. Distal portion of the scleral flap is gripped with further placement in the subscleral space under the formed band of the scleral bed and brought outward, thereby providing fixation of the flap to the scleral bed.

EFFECT: method provides stable hypotensive effect in postoperative period, absence of postoperative astigmatism, physiological adaptation of wound edges, eliminates additional injures of tissues and stimulation of cicatricial processes in filtration zone due to absence of scleral sutures, excludes probability of hyperfiltration of intraocular liquid in postoperative period, reduces the risk of choroid detachment and development of hypotonic syndrome.
