METHOD OF TREATING DEEP OCCLUSION WHEN NARROWING AND SHORTENING UPPER DENTAL ARCH IN CHILDREN WITH ONE-SIDED COMPLETE CLEFT OF UPPER JAW DURING MIXED OCCLUSION Russian patent published in 2020 - IPC A61C7/00 

FIELD: medicine.

SUBSTANCE: invention refers to orthodontics and is applicable in treating children with one-sided complete cleft of upper jaw and deep occlusion with narrowing and shortening of upper dental arch in period of replaceable bite. Removable plate-like orthodontic apparatus equipped with a three-dimensional Bertoni screw with three independent guides for gradual expansion and elongation of the upper dentition and a bite pad in the anterior maxilla is applied. Apparatus is retained by round clasps on canines, Adams clasps on upper first molars. Screw installed in basis of removable device provides slow rate of activation - 0.25 mm per week for 3 months, with active intrusion of lower cutters under action of chewing forces by means of bite pad. Simultaneously teeth of lower tooth row are fixed with braces with nitinol arc of 0.014 inch. In two months, this arc is replaced with 0.016-inch round-nickel-titanium cross-section, and in two more months - at 0.016 × 0.022 nickel-titanium arc with rectangular cross-section. At the end of six months from the beginning of treatment, a steel arc with a rectangular cross-section of 0.016 × 0.022 is applied on the lower dental arch and a ligature binding of the lower dental line is performed. Removable apparatus on upper jaw is cancelled, and teeth of upper jaw are installed brackets equipped with nitinol arc of round cross-section 0.014 inches, which after two months is replaced by nickel-titanium circular section of 0.016 inches, then in two months is replaced by a nickel-titanium arc of rectangular cross-section 0.016 × 0.022. Required position of the teeth relative the cleft of the alveolar process, the width and length of the upper dentition, and the correction of the shape of the upper dental arch are achieved.

EFFECT: method provides normalization of width and length of upper dentition, correction of deep occlusion, and as a result provides creation of conditions for further harmonious development of maxillofacial region.
