FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics, and can be used for surgical treatment of osteoarthritis of first metatarsophalangeal articulation (I MPA), or hallux rigidus. Incision of 4–6.5 cm is performed along a medial surface in a projection of the first metatarsophalangeal articulation. Performing arthrotomy and cheilectomy. Segment of a defect of an articular cartilage from a remains of the affected cartilage is cleared to a subchondral bone. Microfracturing is carried out in the defected section of the defect with a drill or wire of holes with diameter of 0.5 mm with depth of 5 mm uniformly with distance between holes of 2–3 mm in staggered order in rows. Matrix is soaked in physiologic saline for 2–7 minutes. Performing the pattern by applying a sterile transparent suture, outlining the defect and cutting out the resulting pattern, which is congruent to a defect of the articular surface. Required fragment is cut as per the resulting pattern from said matrix. On the defect of the articular surface, the obtained fragment of the two-layer matrix is applied with a porous layer to the bone surface and sutured to the intact articular cartilage or the periosteum with the absorbable sutures. Closing the wound. Patient is verticalized on the following postoperative day. Wearing Barouk shoes to unload forefoot for 6 weeks after the operation, afterwards, wearing ordinary shoes is carried out using individual insoles.
EFFECT: method enables recovering the volume of movements in the I MPA and completely recovering the normal biomechanics of walking by removing the osteophytes of the instep bone head and the base of the proximal phalanx of the hallux and stimulating growth of hyaline-like tissue in the area of the treated cartilage defect.
