METHOD FOR SURGICAL MANAGEMENT OF PATIENTS WITH FREIBERG-KOHLER DISEASE Russian patent published in 2024 - IPC A61B17/56

SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for surgical treatment of patients with Freiberg-Kohler disease. Method involves arthrotomy and debridement of the metatarsal-phalanx joint, a closed wedge osteotomy of the metatarsal bone with resection of the necrotic focus, reduction and fixation of metatarsal bone fragments. During the metatarsal osteotomy, the distal sawing is performed along the plantar edge of the necrotic focus. Then, proximal sawing is performed along the edge of the dorsal border of the necrotic focus. Directions of the saws are chosen so that the saws converge in the area of the distal meta-epiphysis of the metatarsal bone. During the metatarsal bone fragments reduction, the distal fragment is turned to the proximal fragment and displaced distally along the osteotomy line until the Lelievre parabola is restored.

EFFECT: method provides restoration of anatomy and biomechanics in forefoot, creates conditions for reduction of load on heads of adjacent metatarsal bones, prevents formation of transfer metatarsalgia syndrome due to preservation of arc of length of metatarsal bones.
