FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to orthopedics, and can be used in the treatment of metatarsalgia and deformation of the fingers with transverse flatfoot. For this purpose, perform a dorsal access longitudinally over the metatarsophalangeal joint (MPHJ) of the deformed finger, open the metatarsophalangeal joint and secrete the distal epiphysial cartilage of the metatarsal bone at the level of the proposed osteotomy. Perform the first osteotomy, exposing the saw blade parallel to the frontal plane of the metatarsal bone 1 cm proximally from the line of articular cartilage of the head from the rear to the sole of the foot, orient the saw blade at an angle of 50–80 degrees to the horizontal axis of the metatarsal bone. Departing 0.4–0.7 cm proximally from the first osteotomy performed, a second osteotomy is performed parallel to the first, resecting a cylindrical fragment of the epiphysial cartilage. Move the head of the metatarsal bone vertically upward to restore alignment with the base of the proximal phalanx. Metatarsal head is fixed in the position of the achieved correction, for which the Kirchner needle is first carried through the head of the metatarsal bone into its bone marrow channel to the lower cortical layer of the proximal osteotomy fragment. Then, the subchondral layer of the articular surface of the head of the metatarsal and the plantar cortical layer of the metatarsal are drilled with a cannulated drill and the head of the metatarsal bone is fixed to its diaphysis cannulated, self-drilling, compression screw, which is carried out on a previously installed Kirschner spoke.
EFFECT: method provides an effective and reliable elimination of finger deformation and metatarsalgia with the restoration of the physiological ratio of the structures of the MPHJ and its function.
