Abstract RU 2737860 C1

FIELD: medicine.

SUBSTANCE: group of inventions refers to medicine, namely to a method of supporting decision making in emergency cardiac care and a device for its implementation. That is followed by formation of entropy-parametric features of the electric cardiosignal sample (ECS) with selection of a recipe of cardiac cycles. Determining central moments and information features of ECS, as well as such entropy-parametric features as entropy coefficient, asymmetry and counterexcess for distribution of ECS time interval samples and feature spread. Criteria of hemodynamically relevant arrhythmia (HRA) are determined with the possibility of evaluating the truth of the inequality of the difference in the asymmetry feature of the ECS time interval sample count and its minimum critical value with switching of information processing sequence in case of HRA occurrence. Reliability of establishing the ECS rhythm of a condition with severe ventricular disorders (SVD) is determined with forming SVD state domain boundaries. Probability of error is calculated as a result of incorrect statement on establishment of rhythm of pathological condition with SVD. Reliability criterion of the SVD pathological condition is stated by comparing it with its critical value with connection of means of informing on necessity of defibrillation or revascularization. Pathological condition is predicted with definition of an entropy-parametrical criterion for ECS rhythm of an optimum condition. Criterion and its critical value are compared at preset significance level a. Heart rhythm is informed if the criterion is less than the critical value. Pathological condition is predicted if the criterion is equal to or greater than the critical value by determining the parameters of the boundary of the decision area and the coordinates of the rhythm of the optimal state in the space of features of the distribution of ECS counts. ECS rhythm decision making area boundaries are formed. Error probability is determined and compared with a critical value to establish the presence of an independent symptom of the pathological condition of the heart. Need for defibrillation with presentation of audio and visual information and possibility of connection of defibrillator control means.

EFFECT: enlarging functional capabilities of assessment of heart condition on the basis of entropy-parametric analysis of ECS for making a decision on cardiac defibrillations or preparing the patient for immediate myocardial revascularization when detecting shock rhythms; in the absence of hemodynamically significant arrhythmia, a method of supporting decision making based on an analysis of the entropy-parametric criterion formed in the space of the entropic coefficient, counterexcess and asymmetry, enables to establish presence of independent symptom of pathological condition of heart.
