FIELD: medicine.
SUBSTANCE: group of inventions relates to medical equipment, specifically to three versions of the endotracheal tube. In the first version, the endotracheal tube comprises a body made in the form of a flexible curved transparent tube having open proximal and distal ends and convex and concave sides. Tube has a connector installed on the proximal end of the tube for connection of the tube cavity to the apparatus of artificial pulmonary ventilation. Tube comprises an inflatable cuff tightly fixed around the distal end of the bent tube and configured to hold the distal endotracheal tube in the larynx in the required position. Tube contains a longitudinal channel for air supply, which is made in the wall of the bent tube and communicates with the cavity of the inflatable cuff. Tube comprises sealed balloon with resilient walls communicated with air supply port, connected by means of catheter with longitudinal channel for supply of air into cavity of cuff and configured to maintain and monitor pressure in cuff cavity. On the surface of the tube on the side of the proximal end a mark is made at distance of 2.5-3 cm from the cuff. Inflatable cuff is two-layered. Outer layer of mantel is made porous of biocompatible polymer material and serves as depot for introduction and retention in pores of material of medicinal agent and removing the drug from the pores into the area of the mantelock with the trachea contact when the mantle is inflated by the infusion of the drug into the tracheal mucosa. In the second version, the tube does not contain a marking mark, but includes a longitudinal drug delivery channel made in the wall of the bent tube and connected by means of a catheter with a container for dispensing drug delivery. Central angle between longitudinal channel for air supply and longitudinal channel for drug delivery is within 90-180°. On the surface of the tube there are at least two marking marks, one of which is located in the immediate vicinity of the cuff, the second mark is located at a distance of about 2-2.5 cm from the first marking mark, closer to the proximal end of the tube, so that it can be seen from the laryngopharynx in the required working position of the endotracheal tube at the border of the upper larynx. Tube has two channels made in body of bent tube and lying in plane perpendicular to tube axis, extending to the surface of the tube in close proximity to the second mark on the side of the proximal end of the bent tube, communicating with the longitudinal drug delivery channel and ending with the cone-shaped outlet. Tube includes two channels made in body of bent tube and lying in plane perpendicular to tube axis, extending to the surface of the tube in the immediate vicinity of the mantelsee between the first mark and the mantle, in communication with the longitudinal drug delivery channel and ending with the cone-shaped outlet. Tube comprises two channels made in the body of the bent tube and lying in a plane perpendicular to the axis of the tube, extending to the surface of the tube in close proximity to distal end of tube between cuff and distal end of tube, communicating with longitudinal channel for drug delivery and ending with cone-shaped outlet. Upper layer of the cuff is designed for insertion and retention in the pores of the drug material and removal of the drug from the pores into the zone of contact of the cuff with the trachea when the cuff is inflated due to the infusion of the drug into the tracheal mucosa. In the third version, the tube comprises an annular channel for drug delivery to the area of the mantel's contact with the tracheal mucosa and into the lower respiratory tracts, made in the tube wall in the immediate vicinity of the inflatable cuff on the side of the proximal end of the tube, between the cuff and the labeling mark interconnected with the longitudinal drug delivery channel. Tube includes at least three pairs of holes of different orientation, made at a distance from each other along the circumference. Two pairs of holes have cone-shaped shape with larger diameter of cross section closer to external surface of wall and axis of each hole of said pair is arranged at angle of 45° to axis of tube and is directed towards the distal end of tube. Axis of each hole of said second pair is arranged at angle of 90° to tube axis. Third pair of holes has cylindrical shape and the axes of holes of the third pair are located at angle of 25-30° to the longitudinal axis of the tube and directed towards the distal end of the tube.
EFFECT: effective administration of drugs, such as local anesthetics, in a given single dose, low dose of drugs used in general anesthesia, reduced risk of overdose, as well as more even distribution of anesthetic in contact zone of tracheal mucosa with endotracheal tube.
