FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to neurosurgery, and can be used for performing spinal decompression in patients with neurological deficiency and myeloenic encephalopathy by MRI findings in case of degenerative and posttraumatic stenosis of thoracic spinal canal. Method involves performing spinal decompression from the posterior medial approach by laminectomy with preservation of intervertebral joints. That is followed by resection of a vertebral arch fragment and a hypertrophied yellow ligament fragment to a vertebral arch foot boundary creating a posterior compression of the spinal cord and the corresponding anterior compression region. That is followed by medial facetectomy with preservation of 2/3 of the area of intervertebral joints and removal of the yellow ligament fragment from the foraminar opening followed by layer-by-layer suturing of the wound. After disappearance of postoperative soft tissue oedema within surgical approach, anterior spinal decompression is performed by endoscopic transthoracic discectomy. Endoscope is controlled by wedge resection of the posterior one-third of the adjacent vertebral bodies. Then, the intervertebral disc is removed to the posterior longitudinal ligament with preservation of 2/3 volume of the intervertebral disc, which is also resected.

EFFECT: method provides spontaneous decompression without compression and avoiding reperfusion syndrome by staged reduction of spinal cord compression.
