FIELD: medicine.
SUBSTANCE: declared group of inventions refers to medicine and medical equipment, namely to endovascular surgery, and in particular to a stent-graft system and a method for treating infrarenal aneurysms of the abdominal aorta using it. Bifurcation stent-graft system for treating abdominal aortic aneurysm involves an aortic-bifurcation segment, two iliac segments and an additional internal segment. Aortic-bifurcation segment comprises a main tubular body with branched legs on a distal end, made of biocompatible woven material attached to stents and supported by them. Proximal end of the aortal-bifurcation segment is configured to engage with a portion of the aortic aneurysmal extension proximal to the renal arteries, and contains only expanded metal, a frame (for example, a stent), without woven material, but with fixing pins for engagement with aortic wall proximal to renal arteries. Portion of the aortic-bifurcation segment placed in the proximal neck of the abdominal aorta aneurysm is made only of a stent/stents, without a biocompatible woven material, but with fixing differently directed (for example, caudal and cranial) pins on outer surface for engagement with aortic wall. Each of the iliac segments comprises a tubular body made of biocompatible woven material attached to the stents and supported by them. Proximal end of each iliac segment is connectable to a lumen of the aorta-bifurcation segment. Additional internal segment is made with possibility of tight adherence to part of aortic-bifurcation segment arranged in proximal neck of abdominal aorta aneurysm, and comprises a tubular body of biocompatible woven material and stents attached to the biocompatible woven material along the length of the tubular body and supporting it. Method of treating abdominal aortic aneurysm using said bifurcation stent-graft system comprises the steps of: introducing aortic-bifurcation segment into abdominal aorta; locating the aortic-bifurcation segment in contact with aortic tissues so that the metal frame located on its proximal end was attached to the aortic wall proximally to the renal arteries, and a portion of the aortic-bifurcation segment located in the proximal neck of the abdominal aortic aneurysm is coupled with a wall of the proximal neck of the aneurism of the abdominal aorta; introducing a first iliac segment into a left or right iliac artery; placing the iliac segment in contact with tissues of the left or right iliac artery and connecting the proximal end of the iliac segment with a lumen of one of the legs of the aortic-bifurcation segment; introducing a second iliac segment into another right or left iliac artery; placing a second iliac segment in contact with the tissues of the other right or left iliac artery, respectively, and connecting the proximal end of the iliac segment with a lumen of another aortic bifurcation segment; introducing a balloon catheter and performing an additional balloon dilatation of a portion of the aortic-bifurcation segment located in the proximal neck of the aneurism of the abdominal aorta; introducing an additional internal segment into the aortic-bifurcation segment; additional internal segment is placed in close contact with part of aortic-bifurcation segment located in proximal neck of abdominal aorta aneurysm.
EFFECT: technical result provided by the declared group of inventions is improved fixation of the stent-graft to the wall of the neck of the abdominal aorta and higher tightness of the aneurysmal sac from the main lumen of the abdominal aorta.
