FIELD: medicine, thoracic surgery.

SUBSTANCE: invention can be used for extrapleural thoracoplasty in destructive pulmonary tuberculosis. A paravertebral incision is performed with a dissection of trapezius muscle, tendon edge of large rhomboid muscle at edge of spine is isolated and cut along tendon edge without damaging muscle structures. Resection of first rib to cartilaginous part, second rib to midclavicular line, third and fourth ribs to midaxillary line and fifth rib to posterior axillary line is performed. Then it is fixed with U-shaped sutures to intercostal muscles, leaving tendon edge. The tendon edge of rhomboid muscle is deployed under aponeurosis of superior posterior dentate muscle and under edge of muscle that straightens spine, and is strengthened to latter with single U-shaped sutures. Drainage is installed in subscapularis leading to resected cartilage of first rib. Single sutures are applied to intersected area of trapezius muscle, a layered suture of postoperative wound. The drainage is removed 2-3 days after operation.

EFFECT: method provides good cosmetic effect, reduced trauma and effective collapse of lung due to exposure of large rhomboid muscle under aponeurosis of superior posterior serratus muscle and under edge of spine-straightening muscle straightening spine.
