SUBSTANCE: invention relates to medicine and namely to surgery. A skin-preserving mastectomy is performed with a one-stage reconstruction with a silicone implant or a silicone implant in combination with a thoracodorsal flap; or delayed breast reconstruction using silicone implants or autologous flaps; or breast-conserving surgery with the removal of the nipple. The plate of the xenopericardium BioLAB-PP/PA is prepared before the operation. A shaped flap is cut out of it. It is folded into a roll, while its upper and lower petals are folded inside the flap in accordance with the size of the only contralateral nipple. The skin is cut with a shaped three-lobed "C – V flap" incision to the subcutaneous fatty tissue during the operation according to preliminary marking. Skin flaps are separated with a small layer of adipose tissue and recreate the skin frame of the future nipple - the edges of the donor zone are brought together in the area of the nipple base and interrupted sutures are applied with non-absorbable suture material. Further, the lateral "petals" are fixed to each other, the base of the formed cone is fixed to the edges of the reduced donor zone, and a folded flap from the plate of the xenopericardium BioLAB-PP/PA is placed in the formed cavity inside the formed cone of the future nipple. Then the upper part of the nipple cone is covered with the middle "petal" of the skin and fixed with three non-absorbable interrupted sutures.
EFFECT: long-term aesthetic effect and operated and healthy symmetry of the mammary glands are ensured. The quality of life of patients is improved after comprehensive breast cancer treatment. The method is technically simple to use.
