METHOD FOR SINGLE-STAGE RECONSTRUCTION OF NIPPLE-AREOLA COMPLEX WITH LOCAL TISSUES WITH DE-EPIDERMISATION OF SITE WHEN PERFORMING SKIN-PRESERVING MASTECTOMY IN PATIENTS WITH BREAST CANCER Russian patent published in 2024 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to operative surgery and oncology. Stage of skin-preserving mastectomy with a T-invers approach is performed with the implantation of the endoprosthesis. Initial distance to the nipple-areola complex is marked along the middle clavicular line, then a new location of the nipple is marked along the same line at distance of 19-23 cm from the clavicle. At this distance, from the side of the medial meridian, a three-leafed skin and subcutaneous fat are incised towards the median meridian, as shown in Fig. 1. A mastectomy is followed by a simultaneous formation of a nipple, for which a trefoil flap is used, its extreme petals are matched. They are sutured together with separate sutures from non-absorbable material, forming a frame. Remaining central part of the trefoil is fixed with separate interrupted sutures from a non-absorbable material from above to the frame. Formed nipple is fixed to the previously de-epidermised platform at the new location of the nipple.

EFFECT: method enables simultaneous restoration of the nipple with the implementation of the stage of skin-preserving mastectomy with reconstruction with an endoprosthesis, preserving the projection of the nipple in the remote period, improving the aesthetic result and quality of life in breast cancer patients.
