METHOD FOR SELECTING PATIENTS FOR EXTRAPOLATION ROBOTIC MECHANICAL TREATMENT FOR INJURIES OF THE SHOULDER ROTATOR CUFF Russian patent published in 2021 - IPC A61H1/00

Abstract RU 2748130 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, traumatology and orthopedics, medical biomechanics, physical therapy, functional diagnostics, methods of robotic therapy for injuries of the upper limb, it can be used to assess the biomechanical characteristics of the shoulder joint (hereinafter – SJ) for injuries of the shoulder rotator cuff for a differentiated approach to the choice of treatment depending on these characteristics. First, the biomechanical parameters of the SJ are evaluated: torque in Nm, force in N, the volume of each of the four movements in the SJ in degrees from 0° to 90°, for healthy and injured SJ: flexion, extension, abduction, reduction. Subsequently, a graphical model is constructed for each of the four movements of each SJ in the Microsoft Office Excel program, based on the results of the analysis of which two graphs of dependence are built for each of the four types of movement of each SJ - healthy and injured. On the graphs: 1) on the X-axis, the volume of movements in the SJ is marked in degrees, and on the Y-axis, the force is marked in N, 2) on the X-axis, the volume of movements in the SJ is marked in degrees, and on the Y-axis, the torque is marked in Nm. After that, the two graphs of each of the four movements are superimposed on each other and the minimum points on the Y-axis and the ratio of their values in degrees on the X-axis are determined. Next, these ratios are compared for each of the four movements in the healthy and injured SJ. If the values of the ratios for healthy and injured SJ for each of the analyzed movements are in the range of 1:0.5 and higher, respectively, then the patient is considered to be indicated conservative therapy in the form of exercises using the CON-TREX, PHYSIOMED Elektromedizine AG device, Germany, under the control of the mentioned ratios. If the ratio for at least one of the movements - flexion, extension, abduction, reduction, in a healthy and injured SJ, respectively, is lower than 1:0.5, then surgical treatment is considered indicated.

EFFECT: method provides an objective assessment of the functional state of the SJ by biomechanical parameters using the results obtained as criteria for selecting patients for conservative mechanical treatment or for recommending surgical intervention.
