METHOD FOR SELECTING THE DOSE OF UNFRACTIONATED HEPARIN Russian patent published in 2021 - IPC A61B5/26 A61B5/107 A61K31/727

Abstract RU 2748641 C1

FIELD: medicine.

SUBSTANCE: method for selecting the dose of unfractionated heparin (hereinafter – UFH) for prolonged systemic heparinization when performing hemofiltration in a continuous extracorporeal circuit in newborns with multiple organ failure syndrome on a “multiFiltrate” device of the company “Fresenius”, Germany, in the mode of continuous venous hemofiltration CVVH using the program “ped. CVVH”, a set for extracorporeal blood purification “multiFiltrate-Kit paed CRRT/SCUF”, a hemofilter “Ultra-fluxAVpaed” and a solution “MultiBik” of the company “Fresenius”, Germany, as a substitute, includes a study of the blood of a newborn, in the plasma of which the activated partial thromboplastin time (hereinafter – aPTT), s, is determined; the body weight of the newborn (m), kg, is determined; the dose (δ), UNITS, of unfractionated heparin injected into a continuous extracorporeal circuit every hour is determined depending on the values of aPTT by the formulas: δ=10,0×m at aPTT<60, δ=8,0×m at 60≤aPTT<80, δ=6,0×m at 80≤aPTT<100, δ=4,0×m at 100≤aPTT.

EFFECT: invention makes it possible to objectify the choice of the optimal dose of UFH for prolonged heparinization, practically eliminate the number of cases of thrombocytopenia and bleeding, and prevent blood clotting in the extracorporeal circuit.
