SUBSTANCE: invention relates to medicine, namely to oncourology. The method of transurethral resection of a bladder tumor larger than 3 cm in size includes performing a plasma resection of the exophytic part of the tumor with slices at least 5 mm deep, leaving a part of the tumor with a height of 10-15 mm adjacent to the bladder wall, while resection is performed with a bipolar resectoscope loop by movements of the loop in directions. The method includes removing the resected fragments of the exophytic part of the tumor through the lumen of the resectoscope tube; making markings in the form of short cuts in the mucous and submucous layers of the bladder around the remaining part of the tumor with a bipolar loop of the resectoscope, stepping back from the edge of the tumor base 8-12 mm, while the first incision is made behind the tumor. Implementation of the distal incision of the superficial muscular layer of the bladder behind the remaining part of the tumor along the marking of the distal incision to form the distal edge of the resection. The method also includes plasma resection of the rest of the tumor with the base in the form of the adjacent mucous, submucosal and superficial muscle layers in a single block, in which the proximal and lateral incisions of the superficial muscular layer of the urinary bladder and the resection of fibers between the superficial and deep muscle layers approximately to the middle of the base of the tumor in the projection are performed on the plane of resection, followed by resection of the fibers between the superficial and deep muscle layers to the distal edge of the resection with cutting off the rest of the tumor with the base in a single block, while the resection is performed by movements of the resectoscope loop in the antegrade directions. The method involves removal of the resected remaining part of the tumor with the base through the lumen of the resectoscope tube; performing plasma ablation of the resection crater and resection margins. At the same time, during resection and after cutting off the remaining part of the tumor with the base, blood vessels of the bladder wall are coagulated with a bipolar loop, and the marking and plasma resection of the remaining part of the tumor with the base is performed when the bladder is filled, which provides tension to the tissues of the bladder.

EFFECT: method provides controlled resection of a large tumor of the urinary bladder while obtaining the highest quality material for pathomorphological examination with the presence of detrusor elements.
