FIELD: traumatology and orthopedics.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used for the treatment of post-traumatic ankylosis of the joints of the hand. The anterior capsule of the damaged joint is dissected under local anesthesia. A dynamic distraction device for external fixation is applied, for which two pins with a diameter of 1.2 mm are inserted transversely, introducing the proximal pin through the center of the head of the proximal phalanx, and the distal one through the center of the head of the distal phalanx. Both pins on both sides are bent 90°C with a 5 mm offset from the skin. At each protruding end, the needles are formed on a hook, which are connected together in pairs with an elastic band. Then, intraoperatively, 0.5 ml of a preparation of low molecular weight hyaluronic acid is injected into the joint cavity and the area of collateral ligaments. On days 7 and 14, platelet-rich plasma is injected into the mentioned area of collateral ligaments from the medial and lateral sides in a volume of 0.5 ml, which is 1,000,000-1200,000 cells. Starting from the second week, therapeutic measures are carried out, including flexion and extension movements with maximum amplitude both in the damaged joint and in the adjacent joints. The device is removed for a period of 4 weeks.
EFFECT: invention provides a reduction in the treatment time, an increase in the elasticity of the ligamentous apparatus, early activation of the joint and the activation of growth factors with an improvement in tissue vascularization due to the sequence of administration of drugs and the use of an external fixation apparatus.
