SUBSTANCE: invention relates to medicine, namely to plastic surgery. Preoperative marking is performed. At the same time, the dissection continues further along the Chassignac space, with the separation of the attachment of the fascia of the lacteal gland and the anterior leaf of the aponeurosis of the pectoralis major muscle, that is, above the upper border of the breast spot. The pectoralis major muscle is marked for subsequent intersection at the level of the nipple transversely from the medial attachment to the medial border of the dentate muscle, to the upper line of dissection. Continuous nodular sutures fix the posterior surface of the lacteal gland at the level of the lower edge of the areola. The pectoralis major muscle is fully intersected by the cauterus, contracting brings out the basal segments of the lacteal gland in the cranial direction, the second row of sutures is superimposed on the posterior surface of the lacteal gland, from the line of intersection of the pectoralis major muscle to the line 1 centimeter lower than the previous sutures.
