FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to operative proctology and plastic surgery. A teardrop-shaped cut-off skin flap with multi-serrated edges is marked with lateralization from 1/3 to 2/3 of the area in width, as shown in fig.4. Anesthesia is performed, the skin is excised with a surgical blade along the markings, thereby forming the opposite multi-toothed edges of the wound, eliminating the entire inflammatory focus to the fascia of the sacrum in a single block, undermining one of the edges of the wound. The wound is sutured in layers with interrupted sutures, starting from the bottom of the wound. After 2-3 months, lipofilling of the intergluteal groove is performed with a fat emulsion obtained from the patient's adipose tissue, previously purified from blood corpuscles by centrifugation. To do this, under anesthesia at the upper point of the intergluteal groove along the midline, the skin and subcutaneous fat are incised to the fascia of the sacrum and a tunnel 1.5-2.0 cm wide is formed by dissecting soft tissues under the skin along the entire length of the intergluteal groove along the midline sacrum to the lowest point, located 3 cm short of the anal canal. Using a cannula with a side window, which is turned towards the soft tissues, 20-25 ml of the obtained fat emulsion is injected through a syringe until the intergluteal groove appears smooth, after which the opening from the scalpel is sutured.
EFFECT: method allows preventing the formation of rough postoperative keloid scars of the intergluteal groove after previously performed surgical interventions in the sacrococcygeal region for excision of the pilonidal cyst of the coccyx.
