FIELD: medical science.
SUBSTANCE: invention refers to medicine, particularly to surgery. Skin incision lines and staggered needle stick points are marked. When marking the skin, a laser grid is used, which is projected by a laser module mounted on a support above the surgical field. Height of the laser unit is adjusted so that the length and width of one square of the grid projected on skin makes 1.5 cm, whereas the laser grid is oriented so that one of the vertical lines matches the middle of a gluteal fold. One-piece spindle-shaped dissection of the skin with subcutaneous fat and fistulous passages is performed along the lines up to a sacral-coccygeal fascia within healthy tissues. Needle is inserted through the marked points into the skin with underruning the subcutaneous fat and thoracolumbar fascia. Under the control of the laser marking, the previously applied sutures are tightened alternatively with preserving a visually fixed projection of the central vertical line of the laser grid on the median gluteal fold.
EFFECT: invention allows radical cure of patients with epithelial pilonidal fistula, avoiding complications associated with excessive tension of tissues, as well as obtaining satisfactory cosmetic effect – to preserve gluteal fold.
