SUBSTANCE: invention relates to medicine, namely to laser surgery. Contour lines are drawn, bordering the boundaries of the areas of the mucous membrane and tissue structures of the submucosal layer to be vaporized, on the mucous membrane of the anterior, lateral and posterior surfaces of the soft palate and its uvula. First, linear coagulation of the mucous membrane is carried out to indicate the contours of laser vaporization of the mucous membrane with the underlying tissues of the submucosal layer from a point located 2-3 mm proximal to the apex of the posterior surface of the tongue, in the form of two lines diverging to the lateral points of the base of its posterior surface at the semilunar folds. Then, from these points, two symmetrical lines continued in an arc-like manner 3-5 mm higher from the lunar fold in the direction from the posterior to the anterior palatine arch to the right and to the left, passing to the anterior surface of the uvula in the region of its base. Further, the lines continued symmetrically along the lateral edges of the anterior surface of the tongue until they converge at a point located 2-3 mm proximal to the apex of the anterior surface of the tongue. After that, the fringed areas of the mucous membrane of the anterior, lateral and posterior surfaces of the palatine curtain are vaporized with the tissue structures of the submucosal layer of these areas. In the case of a patient with hypertrophy of the palatine tonsils of the III degree and/or the introduction of the upper pole of the tonsils or their additional lobule into the soft palate, they additionally carry out their partial or total laser vaporization. For the implementation of linear coagulation of the mucous membrane and tissue structure of the submucosal layer, defocused radiation of a carbon dioxide laser with a power of 2-3 W with an impact spot, the diameter of which does not exceed 1.3 mm, is used at a linear speed of movement of the area of laser action of 3.0 mm per second. Vaporization of the fringed areas of the mucous membrane of the anterior, lateral and posterior surfaces of the palatine curtain with the tissue structures of the submucosal layer of these areas is carried out by layer-by-layer evaporation by moving the area of laser action of focused radiation of 5-7 W the movement of the beam within the area is not more than 1.0 cm2.
EFFECT: method allows to minimize the occurrence of complications in the early and late postoperative periods, to provide effective prevention of worsening obstructive sleep apnea syndrome in patients, regardless of the severity of the disease.
