SUBSTANCE: invention relates to medicine, namely to cardiovascular surgery. At the preoperative stage, the patient undergoes multispiral computed tomoangiography of the coronary arteries and the location of the intramyocardial segment of the coronary artery is determined. When the myocardial bridge is located in the projection of the middle segment of the anterior interventricular artery for surgical access, left-sided anterior minithoracotomy is performed in the IV intercostal space. With the location of the myocardial bridge in the projection of the distal segment of the anterior interventricular artery - left-sided anterior-lateral minithoracotomy in the V intercostal space.
EFFECT: method allows minimizing the risks of developing wound complications in the postoperative period; reduce intraoperative blood loss; reduce the trauma of surgical intervention; achieve a satisfactory cosmetic effect; reduce the risk of developing infectious complications due to a decrease in the area of the wound surface; preserve the frame function of the chest; create a favorable psychoemotional attitude of the patient to the operation.
