SUBSTANCE: invention relates to medicine, namely reconstructive surgery, and can be used in reconstructive plastic surgery after excision of a malignant soft tissue tumor with resection of bone structures. After removal of the tumor of the lower half of the chest wall, the incision of the skin and subcutaneous adipose tissue is extended down from the costal arch along the rectus abdominis muscle and then arched to the crest of the ilium. Next, dissection of the outer leaf of aponeurosis of the anterior abdominal wall is performed and the external oblique muscle is isolated with a leaf of aponeurosis. The feeding vascular bundles are isolated from 4 lumbar arteries, after which the combined flap is moved to the area of the chest wall defect. The fibers of the oblique abdominal muscle and the aponeurosis leaf are sewn with separate nodular seams to the frame of the chest wall. Subcutaneous tissue and skin in the area of the donor wound are sutured in layers.

EFFECT: proposed method makes it possible to reduce the risk of necrosis of the displaced flap due to a large number of feeding vessels, reduce the traumatic nature of the operation due to the absence of additional incisions, and also preserve the carcass of the anterior abdominal wall without the use of additional materials.
