FIELD: medicine.
SUBSTANCE: nvention relates to medicine, namely to abdominal surgery, oncology and oncoradiology. A transhepatic puncture access to the bile ducts is performed with a metal conductor passing through the tumor stenosis along the true lumen of the common choledochus. Material is taken for histological examination from different sites of tumors of the head of the pancreas and the distal choledochus. Bougienage of the transhepatic puncture channel and tumor occlusion zone is performed. At the same time, percutaneous puncture external-internal drainage of the bile ducts is performed with a trephine biopsy of tumors of the pancreatic head and distal choledochus and antegrade papillosphincterotomy of the major duodenal papilla.
EFFECT: method allows, while maintaining a high efficiency of sampling histological material only under X-ray control with 100% information content, to carry out the prevention of acute postoperative pancreatitis in patients with complex or altered anatomy of the upper gastrointestinal tract, allows one to confine oneself to anesthesia, reduce the radiological load on the patient and medical personnel, reduce the likelihood of postoperative complications, reduce the time of inpatient treatment of the patient.
