SUBSTANCE: invention relates to medicine, namely to neurology, and concerns a method for predicting an unfavorable outcome in the acute period of ischemic stroke. A method for predicting an unfavorable outcome in patients with ischemic stroke includes assessing the severity of stroke in a patient on the NIHSS scale, taking peripheral blood from a patient in the acute period of stroke, isolation of mononuclear cells from peripheral blood, labeling mononuclear cells with monoclonal antibodies to surface antigens present on monocytic myeloid suppressor cells, and taking peripheral blood from a patient is carried out in the first 24-48 hours after stroke, the percentage of monocytic myeloid suppressor cells among mononuclear cells is determined, the indicator of the prognostic model of an unfavorable outcome of ischemic stroke is calculated by the formula: Y=e(-3.42+0.23×Х1-0.16×Х2)/(1+e-3.42+0.23×Х1-0.16×Х2), where Y is an indicator of the prognostic model of an unfavorable outcome of ischemic stroke, X1 is the severity of stroke in the patient according to the NIHSS scale in the first 24-48 hours, X2 is the percentage of monocytic myeloid suppressor cells in peripheral blood in the first 24-48 hours in the patient, and at a value of Y≥0.24, an unfavorable outcome of ischemic stroke is predicted.
