SUBSTANCE: invention relates to medicine, namely to surgery, coloproctology. A monofilament laser light guide is inserted through the primary opening or transcutaneously without expansion of the coccygeal passage. Under ultrasound control, laser treatment is performed with cleaning of the passage and evaporation of altered tissues, pocket cavities and fistulous passages with a diode laser operating in a continuous mode. Laser treatment is performed once at a radiation wavelength of 1470 nm and a power of 12 W, the speed of the light guide along the fistulous tract is 1 mm/s, and the exposure time for each fistulous tract is at least 15 s. Ten days later, the fistulous tracts with surrounding cellular tissue and scar tissues are excised radically on the side of the larger lesion, sparingly on the side of the smaller lesion, with two unidirectional incisions in the form of arcs of circles with equal radii. The skin-subcutaneous flap formed in this way on the side of the smaller lesion is moved to the side of the larger lesion and fixed with an absorbable thread to the wound edge with separate interrupted sutures.

EFFECT: method is highly effective, allows to reduce the risk of recurrence of the disease, to shorten the postoperative period.
