FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely vascular surgery. The internal jugular vein is squeezed, its transverse venotomy is performed. A diagnostic conductor is inserted into the inferior vena cava, through which a thromboextractor (TREX) is inserted, together with a guide catheter, followed by its positioning and opening below the renal veins to the entire lumen of the inferior vena cava. Thrombectomy is performed from the inferior vena cava, common iliac, external and internal iliac veins, common femoral, superficial femoral and deep femoral veins with a Fogarty catheter inserted distally beyond the thrombi. When stenosis is detected in the inferior vena cava, common iliac and external iliac veins, the TREX device is folded and removed with thrombotic masses through the internal jugular vein, washed and positioned along the conductor in the inferior vena cava. A balloon catheter is positioned and inflated along the conductor into the stenosed segment, and pre-dilation is performed. The balloon catheter is replaced with a stent. The stent is positioned and opened in the projection of the stenosed segment, the stent delivery system is changed to a balloon catheter and post-dilation is performed. The balloon catheter is removed and a conductor catheter is inserted along the conductor, a control phlebography of the iliocaval segment is performed. In case of patency and absence of signs of residual stenosis and dissection, the TREX device is folded and removed together with the conductor catheter and the conductor.
EFFECT: method reduces the likelihood of developing post-thrombophlebetis disease and the intensity of its symptoms, eliminates the risk of pelvic fullness with the complete elimination of May-Turner syndrome, provides minimal traumatization of blood vessels and soft tissues during surgery, makes it possible to safely perform thrombectomy with floating, free-lying and occlusive thrombi in the iliocaval and femoral segments.
