FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to vascular surgery. Preliminarily, an additional opening is created in the thromboextractor trap bag (TREX), into which, with the loop closed and the trap folded, a guide catheter is inserted. A mini-access to the internal jugular vein and its venotomy are performed under local anesthesia. Under X-ray control, an angiographic guidewire is inserted into the vessel and passed through the superior vena cava, the right atrium into the inferior vena cava. TREX with a guide catheter installed in it is passed into the inferior vena cava along the angiographic guide. A contrast agent is introduced; a cone-shaped trap-bag is opened, completely blocking the lumen of the vessel. A guide catheter is inserted into the affected segment and a Fogarty catheter is passed through it to the base of the thrombus. Thrombectomy is performed from the iliocaval and femoral segments, the Fogarty catheter is removed. Through the guide catheter, the femoral, iliac and inferior vena cava are contrasted. In the absence of residual thrombosis, the TREX device is folded and removed through the internal jugular vein. The venotomy of the internal jugular vein is sutured with synthetic thread 6/0, hemostasis, drainage into the surgical approach; the wound is sutured in layers.
EFFECT: method allows eliminating the risk of intraoperative pulmonary embolism, reducing the risk of anesthesia, reducing vascular and soft tissue trauma during surgery.
