METHOD OF ENDOVASCULAR THROMBECTOMY IN COMBINATION WITH OPEN NEPHRECTOMY IN PATIENTS WITH TUMOR INVASION INTO THE RENAL AND INFERIOR VENA CAVA Russian patent published in 2023 - IPC A61B17/00 A61B17/22 

FIELD: medicine; endovascular surgery; urology.

SUBSTANCE: through a phlebotomy incision, an angiographic conductor is inserted into the right subclavian vein and advanced into the inferior vena cava (IVC), placing it above the top of the tumor thrombus. A closed thromboextractor with an additional internal loop with hooks is installed in the thoracic region of the inferior vena cava above the cranial end of the floating tumor thrombus and the hood of the thromboextractor is opened. A thromboextractor is put on the thrombus, an additional inner loop with hooks is tightened, and the hood is closed, cutting off the floating thrombus. In this position, the thrombextractor is removed through the phlebotomy opening. An open nephrectomy is performed, marginal pressing of the tumor venous masses into the lumen of the renal vein with its further removal.

EFFECT: method allows to reduce the time of surgical treatment, reduce blood loss during surgical treatment, reduce the number of intra- and postoperative complications, increase the survival rate of patients and the effectiveness of endovascular thrombectomy in combination with open nephrectomy in patients with tumor invasion into the renal and inferior vena cava.
