METHOD OF PLASTY OF A ROTATIONAL MUSCULAR FLAP OF DEFECTS OF A MANUBRIUM OF STERNUM AND A STERNAL EXTREMITY OF CLAVICLE AT A STERNO-MEDIASTINITIS Russian patent published in 2020 - IPC A61B17/00 A61B17/03 

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to reconstructive, plastic and aesthetic surgery, and can be used for plasty of manubrium of sternum defects and sternal end of clavicle with sterno-mediastinitis. It is followed with resection of deformed and osteomyelic affected areas of the sternoclavicular junction and manubrium of sternum, as well as treating the wound edges with an antiseptic solution. Formed defect is eliminated by means of a plasty with a large pectoral muscle flap cut out of a clavicular part of a greater pectoral muscle, taking into account the anatomy of the main blood supply vessels of the intercostal branches of the internal thoracic artery transferred at angle of 120°. Defect of muscular tissue formed at the point of the flap withdrawal is eliminated by displacement to the clavicle of the sternum-rib of the greater pectoral muscle with its subsequent fixation to the clavicle-thoracic fascia. Size of the muscular flap is determined by a trial tamponade of the defect of the manubrium of sternum and the sternal end of the clavicle. Final tamponade of the wound defect is performed by fixing the free edge of the flap to the periosteum located along the edges of the defect.

EFFECT: method allows reducing suppurative inflammation of a portion of a pectoral-clavicular joint and a manubrium of sternum, eliminating paradoxical breathing and underlying pain syndrome, eliminating deep defects of the pectoral-clavicular joint and a manubrium of sternum arising from sterno-mediastinitis of various etiology.
