FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely orthopedics, and can be used to correct the dorsal deviation of the small toes. The method includes performing osteotomy of the metatarsal bone and the proximal phalanx of the small toe in the direction from the back of the foot to the sole to the cortical plate, the formation of a bone fragment and its placement in the zone of osteotomy of the proximal phalanx of the toe. The first step is to perform an intervention on the metatarsal bone, retreating proximally from the border of the cartilaginous part of the head of the metatarsal bone 2 cm. Then, a diaphyseal osteotomy is performed, directing the saw blade from the back of the foot to the sole in the distal direction at an angle of 45 degrees to the axis of the metatarsal bone. After that, the distal fragment of the metatarsal bone is shifted proximally along the plane of the osteotomy, lifting it up by 3 mm, and the bone fragments are fixed with a cannulated screw in the proximal direction from the back to the plantar surface of the metatarsal bone. The second stage on the operating table simulates a static load on the anterior part of the foot and determines the value of the angle of the residual rear deviation of the small toe, which is equal to the value of the angle between the line of the longitudinal axis of the distal fragment of the metatarsal bone and the line of the longitudinal axis of the proximal phalanx of the small toe. Osteotomy is performed perpendicular to the longitudinal axis of the proximal phalanx of the toe, retreating 0.5 cm from the base of the proximal phalanx of the toe, directing the saw blade from the rear to the sole parallel to the plane of the base of the proximal phalanx. Then, the space between the bone fragments in the osteotomy zone is expanded to an angle open to the back surface of the foot, equal in magnitude to the angle of the residual rear deviation of the small toe, bone fragments obtained during correction of the deformation of the first toe are placed in the free space in the osteotomy zone.
EFFECT: method provides an increase in the effectiveness of surgical treatment, reduction of traumatism, prevention of relapses of the disease in the postoperative period, as well as restoration of the statnamic function of the foot, elimination of pain syndrome by taking into account the individual characteristics of the condition of the patient’s small toe.
