FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to surgery and orthopaedics, and can be used for treating hammertoe deformation of the second toe. On the back surface of the second metatarsal bone, two parallel lines are marked: the first line on the proximal border of the cartilaginous part of the head of the metatarsal bone, perpendicular to the longitudinal axis, the second line is located proximally from the first line at a distance of the required metatarsal bone shortening preliminarily calculated from the X-ray image so that the head of the second metatarsal bone is 3 mm higher than the head of the third metatarsal bone. Osteotomy of the second metatarsal bone is performed by placing the saw blade on the first line, in the proximal direction from the rear to the sole at angle of 20° to the back surface of the bone, as a result, two fragments of the second metatarsal bone are formed: distal and proximal. Osteotomy of the proximal fragment of the second metatarsal bone is performed by placing the saw blade on the second line, in the proximal direction from the rear to the sole at angle of 25° to the back surface of the bone. Obtained free bone fragment is removed. Distal fragment of the second metatarsal bone is brought closer to the proximal fragment and moved along the osteotomy plane of the proximal fragment until the proximal border of the cartilaginous part of the head of the metatarsal bone is aligned with the distal edge of the proximal fragment. Head of the second metatarsal bone is displaced in the plantar plane towards the inclination of the proximal phalanx of the second finger by the required displacement value calculated in advance on the X-ray image. Distal and proximal fragments are pressed and fixed with a cannulated screw in the distal direction from the back to the plantar surface of the bone.
EFFECT: method provides more accurate correction of all elements of hammertoe deformation of the second toe, achieving balance in the ratio of the lengths of the metatarsal bones and optimal distribution of the load on the heads of the metatarsal bones, restoration of congruence and movements in the metatarsophalangeal joint, improved statodynamic function of foot due to required shortening of second metatarsal bone, lifting and lateral displacement of its head by pre-calculated value.
