FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely maxillofacial surgery; it can be used for the manufacture of a multicomponent osteogenic graft in surgical elimination of congenital and acquired defects of the jaw bone. Autologous cell pool of a stromal vascular fraction is prepared in a following way: by a tumescent lipoaspiration method, 200 ml of adipose tissue is taken from the area of the anterior abdominal wall, adipose tissue is washed with a phosphate-salt solution, subjected to enzymatic treatment with a type 1 collagenase solution, after which cells of the stromal vascular fraction are isolated, which are placed in sterile culture Petri dishes with a growth medium, and then, placed in a CO2 gas incubator for cultivation at a temperature of +37°C and a CO2 content of 5% for 7-14 days until reaching a monolayer, after which cells are passed 2-3 times until reaching 50 million cells. The patient has a puncture of the cubital vein, peripheral blood is taken in a volume of 40 ml into 4 tubes with K2 EDTA, after which fibrinogen of autologous blood plasma and platelet mass are prepared in the following way: tubes are centrifuged twice: initially, to settle erythrocytes and leukocytes, then, blood plasma is taken from 4 tubes to a separate tube to obtain fibrinogen of autologous blood plasma of the patient, then, the tube is repeatedly centrifuged to obtain 6 ml of platelet mass. Osteoconductive bone plastic material of animal origin is added to 2 ml of fibrinogen of autologous blood plasma of the patient, the resulting mixture is degassed by immersion into a physiological solution at a temperature of +37°C twice for 20 min each time in a thermostat. Next, autologous cell pool of the stromal vascular fraction is introduced in a volume of 15 million of cells, the, platelet mass is introduced, 200 mcl of a sterile calcium gluconate is added, after which the resulting mixture is incubated at a temperature of +37 degrees for 15 minutes until a fibrin clot is formed, with further packaging in a sterile vial, labeling, and transportation to an operating room in a thermal container at a room temperature. In the operating room, 40 min before use for surgical elimination of congenital and acquired defects of the jaw bone, a dental implant is inserted into the vial with the above-mentioned mixture, while the multicomponent osteogenic graft includes: dental implant – 10 vol.%, autologous cell pool of the stromal vascular fraction – 10 vol.%, platelet mass – 5 vol.%, fibrinogen of autologous blood plasma of the patient – 20 vol.%, granular osteoconductive bone plastic material of animal origin – 55 vol.%.
EFFECT: method provides for the elimination of risks of treatment complication, reduction in material costs, and reduction in a rehabilitation period of a patient due to the elimination of uncontrolled change in a shape and volume of a graft in a recipient’s bed in the postoperative period.
