METHOD FOR FORMATION OF PANCREATOGASTROANASTOMOSIS DURING PANCREATODUODENAL RESECTION Russian patent published in 2022 - IPC A61B17/11 A61B17/00 A61M25/01 A61L17/06

Abstract RU 2779006 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely abdominal surgery. The pancreas is crossed in the body area to form a stump, which is sutured with 4 separate nodular sutures, leaving their ends gathered in a bundle. Drainage is installed into the pancreatic duct with a length of 40-50 mm. At the border of the body and the antral part of the stomach stump, a transverse incision is made corresponding to the length of the diameter of the pancreas equal to 35-40 mm. The stump of the pancreas with drainage of the duct is invaginated to a depth of 10-12 mm into the stomach cavity by pulling the ends of the stitched threads collected in a bundle, while the drainage of the pancreatic duct is brought into the stomach cavity by no more than 20 mm. The pancreatogastric anastomosis is formed by a continuous seromuscular suture, retreating 4-5 mm from the edge of the cut of the pancreas and the stomach wall. A bundle of threads is cut through the lumen of the unsutured stump of the stomach, prepared for gastrojunoanastomosis. Gastrojunoanastomoses and hepaticoejunoanastomoses are formed on one loop of the jejunum in front of the colon. The oral isolated end of the jejunum in the form of a suspended enterostomy is used to place a probe with a diameter of 5 mm distal to the gastrojunoanastomosis by 80-100 mm.

EFFECT: method increases the reliability of the anastomosis, the effectiveness of surgical intervention by eliminating traumatization of the soft parenchymal tissue of the pancreatic stump, provides a reduction in the volume of the pancreatic stump in the lumen of the stomach to accelerate its epithelization.
