FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to surgical dentistry, and is intended for use in bone grafting of the alveolar part of the lower jaw by the method for controlled expansion of the alveolar ridge by splitting it. A local anesthetic is administered. An incision is made in the mucous membrane to the bone along the top of the alveolar ridge and along the edges of it, two vertical incisions limiting the surgical field to the transitional fold. The mucoperiosteal flap is exfoliated and the outer surface of the alveolar part of the lower jaw is exposed. To split the alveolar ridge, a template is used that contains a kappa for installation on the alveolar part of the lower jaw, at least one hole for fixing with a pin to the jaw, and at least one overlay for installation on the crown part of the tooth. The kappa is equipped with a longitudinal bar with a guide incision for osteotomy of the alveolar ridge and three guide incisions for lateral and apical osteotomies. The width of the guide incision for osteotomy of the apex of the alveolar ridge is from 1 to 2 mm and does not exceed half the width of the longitudinal beam. Three guide incisions for lateral and apical osteotomies are made on the lateral surface of the kappa in the form of a U-shaped contour, the sides of which are oriented to the longitudinal beam at a right angle or in the form of a U-shaped contour, with the sides oriented to the longitudinal beam at acute angles between sides and a jumper of the U-shaped contour.
EFFECT: use of a template, due to preliminary digital planning of the location of osteotomies, depending on the location of important anatomical structures, the parameters of the atrophied alveolar ridge, allows for maximum accuracy of the bone grafting operation, reduces its duration, minimizes bone loss, reduces trauma, and the risk of postoperative complications.
