FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to surgical dentistry, plastic reconstructive surgery, and is intended for use in maxillofacial surgery for restoration of bone tissue of an alveolar process of the upper jaw and soft tissues of gums in the area of gingival recession. Mucous membrane of an alveolar process is incised within a gingival recession. Tooth or a group of adjacent teeth with Miller class IV gum recession involved in the operation are preliminarily removed. Thereafter, the mucous membrane of the alveolar process is incised along the alveolar crest to the bone with boundaries in the projection of the extracted teeth involved in the operation and vertical incisions on the left and right along the incision boundaries. Then, exfoliating the mucous membrane of the alveolar process to the bone from the vestibular side within the boundaries of the incisions, a full-thickness mucoperiosteal flap is formed to a level below the apical part of the roots of the extracted teeth. Level of the apical part of the roots of the extracted teeth is determined by CT in accordance with the state of the repaired bone tissue. Flap mobilized by vertical incisions on the left and on the right along the incision borders is folded up, then the bone from the palatal side of the alveolar process is exposed along the incision of the alveolar crest. To the exposed palatal side of the alveolar bone, the edge of the resorbable membrane is fixed with non-resorbable pins with a length corresponding to the length of the incision, bone material is laid on the fixed membrane in an amount sufficient to restore the width and height of the bone tissue volume of the alveolar process in the area of the roots of the extracted teeth, after which the opposite edge of the resorbable membrane is fixed with non-resorbable pins on the exposed bone of the vestibular side of the alveolar process at the level of the detachment boundary of the full-thickness mucoperiosteal flap. Thereafter, the membrane with bone material within the alveolar crest is shaped to the initial anatomical shape of the patient’s alveolar crest. Then on the folded mucoperiosteal flap of the alveolar process with non-resorbable sutures are sutured together the edges of the adjacent sections of the gingival mucosa subject to recession. Thereafter, a resorbable membrane for regeneration of soft tissues “Mucograft” is attached to the inner surface of the sutured sections of the gingival mucosa with non-resorbable sutures. Thereafter, the mobilized folded mucoperiosteal flap of the alveolar process is pulled on the membrane with bone material and sutured without tension with the mucoperiosteal flap of the palatal side with non-resorbable sutures.
EFFECT: by eliminating atrophy of bone tissue of the alveolar process within the Miller class IV gum recession, the method enables to simultaneously restore the volume of the lost bone tissue and eliminate gingival recession, reduce injuries, increase safety and physiologic effect, besides, the method is simplified.
