FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to pulmonology, and can be used to monitor the cardiotoxic effect of bedaquiline in older children and adolescents with respiratory tuberculosis. An ECG is obtained and analyzed with the determination of the corrected QT interval in the supine position and standing before the start of therapy with bedaquiline (Bdq) for the first month - 1 time per week, and then 1 time per month, the results of daily Holter ECG monitoring are performed and analyzed with the determination of the average day/night and daily heart rate, minimum and maximum heart rate, QT/QTc interval values at maximum and minimum heart rate, the duration of the average corrected QTc interval per day, the duration of rhythm pauses, the presence of concomitant arrhythmias and conduction disturbances before starting Bdq therapy. After 3 weeks of treatment, when registering a prolongation of the QTc interval>460 ms on the ECG in the supine position, repeated daily Holter ECG monitoring is performed with a dynamic assessment of heart rate parameters, the duration of QT and QTc intervals, manual assessment of QT and QTc intervals at the minimum and maximum heart rate and when identifying values of the QTc interval<450 ms. According to the manual assessment of the QTc interval at the minimum/maximum heart rate and the average corrected QT interval, after which a decision is made on the use of Bdq at a dosage of 400 mg and on the contraindication to prescribing Bdq with an average QTc>470 ms.
EFFECT: method provides an increase in the effectiveness of treatment by monitoring the cardiotoxic effect of Bdq.
