SUBSTANCE: invention relates to medicine, namely to ophthalmology. Vitrectomy is performed through three ports, two of which 25G for manipulation of surgical instruments are set at 2 and 10 o'clock, the third port is set depending on the eye in the lower outer segment, for the right eye at 8 o'clock, for the left eye at 4 o'clock. The posterior hyaloid membrane of the vitreous body is peeled off and removed using a vitreous cutter. Fibrovascular tissue is removed with a working instrument and a vitreotome bimanually, hyaloid tissue and hemorrhages are removed from the extreme periphery of the retina with a sclerocompressor and vitreotome. Endolasercoagulation of the retina is performed along the vascular arcades and on the extreme periphery, tamponade of the vitreal cavity and transconjunctival sutures are applied at the site of the ports. At the same time, the third port 25G is used for a cannula combined with an illuminator with a supply of a balanced irrigation saline solution, the removal of the vitreous body with the separation of the posterior hyaloid membrane from the retina is carried out with a frequency of cuts of 7500-10000 per minute, a vacuum of 650 mm Hg. As the vitreotome approaches the retina and fibrovascular tissue, the vacuum is reduced to 500 mm Hg. Then, the fibrovascular tissue is dissected and removed using a working instrument and a vitreotome, using the “instant cuts” mode, in which vacuum and cuts work separately. In a particular case, tweezers, or scissors, or an aspiration cannula are used as a working tool.

EFFECT: method allows to reduce the invasiveness of surgical intervention with high rates of treatment efficiency.
