FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely obstetrics and gynecology. An ultrasound examination is performed on a patient with a uterine scar in the 3rd trimester and the risk parameters of hypotonic bleeding are determined: the coefficient of asymmetry of the thickness of the placenta (C), as the ratio of the thickness of the placenta at the level of its upper third (U, mm) to the thickness of the placenta at the level of its lower third (L, mm). The size of the minimum thickness of the uterine wall in the placenta localization area (T, mm). The outer contour of the uterine wall in the area of the CM scar is evaluated, in the presence of deformation: swelling of the outer contour of the uterine wall in the area of the CM scar it is considered equal to 0.5, in conventional units, in the absence of deformation: swelling of the outer contour of the uterine wall in the area of the CM scar it is considered equal to 1, in conventional units. Using the obtained indicators, the risk of hypotonic bleeding in the early postoperative period (EB) is calculated according to the original formula. And with a PK of less than 3.0, a high risk of hypotonic bleeding is predicted. With EB equal to 3.0 or more, there is a moderate risk of hypotonic bleeding in women with a uterine scar.
EFFECT: invention provides a high accuracy of the prognosis of hypotonic bleeding in the early postpartum period in this category of women, which, in turn, provides timely determination of preventive measures, including the choice of delivery method, methods for intraoperative hemostasis, the readiness of all support services for the immediate provision of specialized, including high-tech, medical care.
